New Patient Paperwork

Without the completed forms, the slot you booked online may be taken by someone else who has completed the forms. Please fill out the forms to secure your appointment time. If you choose to walk in to complete the forms, they will be done on the retail side at Lenscrafters, and your appointment may be delayed for up to one hour.

• Insurance Information (Subscriber’s Full Name & Date of Birth) Medical and Vision
• Recent prescriptions for glasses/contacts (If available, please send them over before your appointment)
• Cancellation Policy: We kindly ask for at least 48 hours’ notice for cancellations or reschedules. A $50 fee will apply if we do not receive timely notice.
